USC IP policy requires USC faculty, staff and students to disclose inventions and other forms of intellectual property under certain conditions. From the information provided on the disclosure, the USC Stevens Institute team can properly prepare the right path for your specific needs.
The disclosure form requires specific information including:
- Inventor(s) contact information
- Funding grant and account information
- Dates linked to the invention's discovery
- Pubic disclosure dates
- A complete description
After you disclose your work, USC Stevens Institute will determine the stage of the innovation or project and what additional steps should be taken. Full disclosure will also help our team determine if there may be events that could impact patentability and other forms of protection, or compromise potential rights in some way.
You can fill out a disclosure online and email it to us at: disclosures@stevens.usc.edu or or print the form and mail it to us via campus mail at MC ATT-2300.
Please follow these steps to fill out the form:
1. Download the Microsoft Word version of the form by clicking on this link. FOR SOFTWARE, please download and complete the "Software Addendum" form in addition to the disclosure form.
2. Fill out the form in Microsoft Word. Please use 10 or 12 point font and avoid additional formatting, such as bold, italic, underline.
3. Collect or create supporting documentation such as a written description of the invention, your draft or published manuscript, or related publications by other researchers.
4. Print the form, sign it and send or deliver it to our office (disclosures@stevens.usc.edu / mail code ATT-2300), along with any supporting documentation.
5. If you email the form, mail the signed original form to the USC Stevens Institute (mail code ATT-2300).
